Who Can Refer?
Anyone can make a referral to TishAbility, including:
- • Family members and carers
- • Social workers, local authority teams and commissioners
- • Teachers and school staff
- • Health professionals (nurses, therapists, doctors)
- • Other professionals (youth workers, support coordinators)
- • Voluntary sector organisations
- • Commissioners and local authorities
What Information We Need
To process a referral, please provide:
- 1. Person's details: Name, age, primary support needs
- 2. Support needed: What specific support would help
- 3. Current support: What support is already in place
- 4. Contact information: Best way to reach the family or referrer
What Happens After Referral
Initial Discussion and Scope Check
We will contact the family or referrer to understand what support is being requested, whether it is within TishAbility's non-regulated remit, and whether we may be a good fit.
Planning
If a good match, we'll work with the person and family to plan support that fits their needs and goals.
Delivery
Support begins, with regular communication with the person, family and other professionals.
Review
We regularly review support with the person and family to ensure it's working and adjust as needed.
Funding & Commissioning
Support may be arranged through Direct Payments, Personal Health Budgets, Education Budgets, family-funded arrangements, local authority arrangements, NHS or ICB arrangements, commissioner-funded support, spot purchase or partnership arrangements. We're happy to discuss how support can be funded.
Confidentiality
We take confidentiality seriously and will only share information with relevant parties with appropriate consent. Family data is stored securely and in compliance with GDPR.
